After winning Innovation Incubator grant at Perkins&Will, my colleague and I explored Post-acute care continuum, its state at present and further developed this research to advocate for a holistic patient approach to close the ‘realized’ gap.

This study, through qualitative interviews with subject matter experts, aims to provide an overview of the major trends and business models of innovative players emerging in the post-acute care market. Perkins&Will, through its advisory services, can help in driving this meaningful change. The study serves as a point of departure towards reimagining post-acute care needs within the care continuum and advocates for a holistic approach to patient experience across various health environments- from care facilities to the patient’s home.
